An item may carry a drift note — what a coverage scan of the target group named in distributionList observed about that name:

{ "label": "LoyaltyTier", "insertText": "LoyaltyTier", "kind": "Column",
  "detail": "varchar(20)",
  "drift": { "missingFrom": ["sql-prod-03.Sales", "sql-prod-09.Sales"], "scanned": 12, "typeVariants": [] } }

A note is always relative to how many databases were actually read.

No note means nothing was observed to differ — never that the name is present everywhere. A scan that never ran produces no notes at all, and so does a scan in which everything agreed. Those are different facts and drift: null does not distinguish them; schemaReadUtc and a fresh sqlmc_list_coverage call do.
